Individual Psychotherapy: $225 / 60 minutes
Psychological Assessment/Evaluation: $225 / hour
Professional Consultation: Please Inquire
Court / Legal Services: $250 / hour
Document Preparation: $200 / hour (or fraction thereof)
INSURANCE
PPO plans under any insurance carrier can be used for services, and reimbursement is based on plan benefits. Please contact your insurance carrier for details about your benefits. Dr. Chung will gladly provide superbills for services rendered, upon request.
POLICIES
Late Cancellation / No Show
Any missed appointments that are not cancelled within a strict 24 hour period prior to a scheduled appointment are subject to a $100 fee. Appointments cancelled within 2 hours of appointment will be charged at the full cost of your appointment.
Payment
Accepted forms of payment: Venmo, Credit/Debit Card
Bill Collection
Dr. Chung reserves the right to forward delinquent account balances to bill collection services. If you are experiencing any financial hardships, please contact Dr. Chung directly to discuss special arrangements in order to avoid third party bill collection.
